The Nursing and Midwifery Council of Nigeria is collating the data of all professional Nurses and Midwives that have exited from government employment in Nigeria from 2018 till date.

In view of the above you are hereby directed to provide the required detailed information of all Nurses and midwives that have resigned or exited their services from your institution from January 2018 till date.

Please note that the information should be provided using the attached format in excel sheet, kindly adhere strictly to the attached template and do not send a scanned excel sheet document.

  1. Name of Nursing personnel
  2. Registration number (e.g. RN1234 or RM 1234)
  3. Post on exit (e.g. NO I, NO II, SNO…. Etc)
  4. Cadre/Specialty
  5. Year of Graduation
  6. Date/Year he or she exited
  7. Reason for exit (Statutory Retirement; Voluntary Retirement; Transfer; (Relocation; Abscondment; Dismissal; Deceased)
  8. Previous Employer
  9. Present workplace (Nigeria or outside)
  10. Email Address
  11. Telephone Number

The information should be forwarded to the Registrar on or before March 25, 2020 using the following email addresses info@nmcn.gov.ng and nofilade@nmcn.gov.ng
